E’ ancora una bozza che sarà valutata nei prossimi giorni dalla Conferenza Stato-Regioni, ma il testo del Piano nazionale di Prevenzione vaccinale 2016-2018 risolve il ‘giallo’ sulle possibili sanzioni o addirittura delle radiazioni per i medici restii a vaccinare gli assistiti che ha fatto infuriare i camici bianchi. Il caos mediatico ha richiesto il chiarimento del ministro della Salute che ha sottolineato come ogni “azione in questo senso sarà concordata con gli Ordini”. Ma cosa prevede il Piano nazionale vaccini? Nella bozza si evidenzia come per il percorso “per i vincoli normativi e l’obbligatorietà delle vaccinazioni sarà approfondito e dal nuovo piano potrà essere generata una normazione aggiornata con misure correlate, come, ad esempio, l’obbligo di certificazione dell’avvenuta effettuazione delle vaccinazioni previste dal calendario per l’ingresso scolastico”. Per quanto riguarda la possibilità di sanzioni per i medici poco inclini all’immunizzazione dei pazienti “parte integrante di questi vincoli normativi sarà anche la ricognizione continua delle possibili violazioni del supporto alla pratica vaccinale e dell’offerta attiva delle vaccinazioni da parte dei medici e del personale sanitario dipendente e convenzionato con il Servizio sanitario nazionale”. Quindi ogni tipo di ipotesi di ‘multa’ per il camice bianco sarà dunque concordata con le categorie dopo un confronto con il ministero.
Il costo complessivo dei vaccini inseriti nel calendario vaccinale, secondo il prezzo corrente, a regime e con il raggiungimento dei tassi di copertura presentati più avanti viene stimato intorno a 620 milioni di euro. Si prevede l'”attivazione di un fondo nazionale per i vaccini che possa co-finanziare le regioni in difficoltà oggettive a garantire, ancora una volta, l’uniformità, l’equità e l’universalità dell’offerta su scala nazionale”. Inoltre al calendario attuale vengono aggiunti i vaccini contro il meningococco B, l’antipneumococco oltre i 65 anni, l’antipapillomavirus per ragazzi e ragazze undicenni. Ecco gli obiettivi del Piano Nazionale della Prevenzione Vaccinale 2016- 2018: 1) Mantenere lo stato polio-free 2) Raggiungere lo stato morbillo-free e rosolia-free 3) Garantire l’offerta attiva e gratuita delle vaccinazioni nelle fasce d’età e popolazioni a rischio indicate anche attraverso forme di revisione e di miglioramen- to dell’efficienza dell’approvvigionamento e della logistica del sistema vaccinale aventi come obiettivo il raggiungimento e il mantenimento delle coperture descritte più oltre 4) Aumentare l’adesione consapevole alle vaccinazioni nella popolazione generale anche attraverso la conduzione di campagne di vaccinazione per il consolidamento della copertura vaccinale 5) Contrastare le disuguaglianze, promuovendo interventi vaccinali nei gruppi di popolazioni marginalizzati o particolarmente vulnerabili 6) Completare l’informatizzazione delle anagrafi vaccinali interoperabili a livello regionale e nazionale tra di loro e con altre basi di dati (malattie infettive, eventi avversi, residente e assistiti); 7) Migliorare la sorveglianza delle malattie prevenibili con vaccinazione; 8) Promuovere, nella popolazione generale e nei professionisti sanitari, una cultura delle vaccinazioni coerente con i principi guida del presente Piano, descritti come ’10 punti per il futuro delle vaccinazioni in Italia’ 9) Sostenere, a tutti i livelli, il senso di responsabilità degli operatori sanitari, dipendenti e convenzionati con il Servizio sanitario naizonale, e la piena adesione alle finalità di tutela della salute collettiva che si realizzano attraverso i programmi vaccinali, prevedendo adeguati interventi sanzionatori qualora sia identificato un comportamento di inadempienza; 10) Attivare un percorso di revisione e standardizzazione dei criteri per l’individuazione del nesso di causalità ai fini del riconoscimento dell’indennizzo, ai sensi della legge 210 del 1992, per i danneggiati da vaccinazione, coinvolgendo le altre istituzioni competenti (ministero della Difesa); 11) Favorire, attraverso una collaborazione tra le istituzioni nazionali e le società scientifiche, la ricerca e l’informazione scientifica indipendente sui vaccini.
